New to Phusion Wellness? Here are few common questions that we’ve answered to make your visit more comfortable.

What do I need to bring to my appointment?

If you are planning to visit Phusion Wellness, please bring the following items:

  • Insurance card
  • New patient paperwork (completed online)
  • Records and imaging
  • Medical marijuana card (if applicable)

How do I become a patient?

To become a Phusion Wellness patient, it is required that you provide a medical diagnosis complete with imaging and records.

When do I pay for treatment?

Payments can either be done in office or online at onpatient.com. Because your follow ups will primarily be done remotely, patients are required to pay online through their OnPatient account at least 24 hours in advance of their next appointment.

I am on a high amount of medication, will you still see me as a patient?

Patients that are on a high amount of medication are treated on a case by case basis. Schedule a free consultation with one of our providers to discuss possible treatment options.

Do you prescribe benzodiazapenes?

Phusion Wellness is dedicated to the well-being of our patients. Due to the high overdose risk when taking benzodizapenes and other medication together, we do not prescribe benzodiazapenes.

What insurances do you accept?

Phusion Wellness is not contracted with any health insurance companies and does not accept health insurance. Services are provided on a self-pay basis.

Do you prescribe Somas?

We do not prescribe Somas at Phusion Wellness.

Do I need a referral?

You do not need a referral for Phusion Wellness since we do not take insurance.

How do I transfer my records to your office?

If you would like to become a Phusion Wellness patient, we will need permission to access your medical records. In order for this to happen, please download and fill out the following authorization form. Once you have completed and signed the authorization form you have two options:

  1. You can send the completed form directly to your previous providers and/or specialists.
  2. Or you can send us the completed form and we will send it out to your previous providers.

What is your policy for non-compliance?

Incidents that are considered non-compliant include, but are not limited to, the following:

  1. Abnormal drug screens
  2. Inconsistent pill counts
  3. Sponsor concerns
  4. Inappropriate filling at pharmacy
  5. Lack of documentation

If during treatment you are ever considered as non-compliant regarding the Phusion Wellness policies and protocols, you will be required to complete the follow steps:

  1. Pass random compliance testing (drug screen, pill count, sponsor call)
  2. Participate in behavior health education.
